Features of Kkeut Korean Medicine Clinic's Saesalchim (Subcision) Treatment
We guide you through the three-step approach of Kkeut Korean Medicine Clinic's Saesalchim treatment.
Step 1: Separation of Superficial Adhesions
Fibrous adhesions widely spread beneath the skin surface are precisely cut and removed using fine needles.
By delicately separating the fibrous tissues that pull the scar downwards,
it helps to naturally improve the depressed areas on the skin surface.
Step 2: Removal of Deep Adhesions
Hard adhesive tissues located deep within the dermis layer
are separated using a cannula, reducing strain over a wide area.
This method causes less skin irritation and discomfort compared to the old method of using thick needles.
It can be applied to manage wide, rolling scars or boxcar-type scars.
This step can contribute to reducing discomfort such as bruising or pain.
Step 3: Induction of Tissue Regeneration (using microneedles)
In the final stage, microneedles are used to induce a small bleeding response in the skin tissue.
This creates an environment where blood flow and various growth factors can concentrate in the affected area.
This process can help stimulate fibroblasts to promote collagen production.
Complementary Management Methods During Saesalchim Treatment
We perform complementary treatments to ensure satisfaction after the procedure and promote rapid skin recovery.
① Concurrent PDRN Injection (DNA Injection)
PDRN injections are applied concurrently during the Saesalchim treatment.
This aids in wound healing and can contribute to reducing the likelihood of redness and hyperpigmentation.
As an element that supports the skin tissue regeneration process, we include it as a basic step in our procedures.
② Concurrent Erbium YAG Fractional Laser for Faster Recovery
Subcision treatment focuses on structural changes in the underlying layers of the skin.
However, there may be limitations in improving the skin texture or fine details on the surface.
Therefore, Erbium YAG fractional laser is often applied concurrently after the treatment.
This treatment works at a shallow depth to help improve skin texture, pores, and fine depressed areas on the surface.
Through this, it can reduce the burden of recovery and contribute to increasing overall treatment satisfaction.

we meticulously plan from the diagnosis stage before treatment
which skin layers have adhesions and which direction to approach.
Precise Use of Equipment
By combining various types of needles, cannulas, and microneedles,
we aim for satisfactory improvement while minimizing discomforts such as bruising, swelling, and pain.
Concurrent PDRN and Fractional Laser Treatment
Complementary treatments that help skin recovery and improve the overall results of the procedure
are considered an essential part of the treatment process at our clinic.
Diagnosis and Treatment Recommendation Considering Individual Condition
Subcision treatment is not suitable for all scars.
For scars that do not require treatment, we do not recommend procedures and provide honest guidance.
As medical professionals, we believe that an accurate medical judgment must precede any recommendation for a procedure.
What is New Skin Needle Treatment?
Scars have a characteristic of pulling tissue downwards within the skin.
While they appear indented on the surface, in reality, beneath the dermis layer,
fibrous tethers are firmly pulling down the scar area.
These adhesive tissues must be released for the skin to return to its original position.
The procedure that improves scars by removing this adhered structure is New Skin Needle, or subcision.
Principle of New Skin Needle Treatment
1. Adhesion Tissue Release (Release)
The fibrous bands firmly located within the skin
are incised using a needle or cannula to release the adhered parts.
→ By releasing the adhesions that pulled the skin down, indented scars can be expected to naturally fill in.
2. Inducing Micro-bleeding (Regeneration Signal)
Micro-bleeding that occurs during the process of separating adhesive tissues
serves as an important signal to promote new collagen production.
→ We promote fibroblast activity, creating an environment where new tissue can regenerate in the scar area.
3. Enhanced Blood Supply and Tissue Regeneration
If necessary, microneedling or PDRN (DNA injection) may be applied concurrently
to intensively stimulate blood circulation and regenerative cell activity in the affected area.
This can promote wound healing and collagen production simultaneously.
When can New Skin Needle Treatment be considered?
Deeply indented scars on the forehead area, as if pricked.
Scars where the temple area appears sunken and the skin surface is uneven.
Indented scars on the nose area, appearing as one, two, or several dots.
Complex, deeply indented scars of various forms on both cheeks.
What is Multi-Subcision Treatment? (A 3-step procedure combining traditional New Skin Needle with Er:YAG Shoulderling)
This is a treatment method where the Shoulderling technique is added to enhance the effectiveness of traditional New Skin Needle (subcision).
In scar treatment, there are other aspects to consider besides releasing the underlying adhesions.
Even if underlying adhesions are released with subcision, scars with clear, deeply indented borders may have difficulty regenerating skin uniformly.
Our body tends to heal damaged tissue by matching its height and texture to the surrounding skin.
However, if the scar's border is abruptly steep, this natural healing process can be difficult to achieve smoothly.
Therefore, we also perform a Shoulderling procedure that finely refines the scar's borders into a gentler, more gradual shape.
When the borders are softened in this way, the height difference with the surrounding skin is reduced, and when the scar fills in, it can be expected to look more natural and less noticeable.
Many individuals have not experienced sufficient improvement with traditional New Skin Needle treatment alone, and in some cases, these border issues are the cause.
Even if adhesions are released, if the scar's border remains high like a wall, the skin regeneration process may not proceed smoothly.
Therefore, Shoulderling is a process that helps to complement the limitations of existing treatments for indented scars with distinct borders.
Cases where Multi-Subcision Treatment can be considered
1) Cases where satisfactory improvement was not achieved with traditional New Skin Needle treatment or other general scar treatments.
Even if you have already received subcision or laser treatment, if the scar has not filled in as much as expected, you may still feel unsatisfied with the improvement.
This is because simply releasing the adhesions under the scar prevents the skin from filling in naturally when the edges remain steep.
For such scars, through a shouldering procedure, gently smoothing the edges can lead to better improvement.
2) It can also be applied to scars that are deeply indented, with clear and steep borders.
Boxcar scars (abruptly depressed scars) are a type where, even if the skin tries to fill in from below,
the side borders remain firm like a wall, sometimes hindering complete recovery.
When the steep edges of a scar are gently reshaped with Er:YAG shouldering, the difference in height with the surrounding skin is reduced, allowing for a more natural transition.
3) Recommended for those who desire effective scar improvement with fewer sessions than 'saesalchim' (new skin needling) treatments.
Traditional 'saesalchim' often requires consistent sessions over multiple treatments for effects to gradually appear. In contrast, multi-layered 'saesalchim' approaches by simultaneously treating the adhered base, the steep side borders, and the surface of the scar.
Through this comprehensive approach, you can experience changes in scar improvement with fewer sessions compared to traditional 'saesalchim' treatments.

Q&A
고객님들이 가장 많이 물어보시는 내용을 모았습니다.
How much pain is involved during the new skin needle treatment?
Discomfort during the procedure can vary from person to person, but generally, many feel a sensation similar to acne extraction. To minimize pain, we apply sufficient numbing cream before the procedure.
What is the recommended number of treatment sessions?
Generally, we often recommend about 10 sessions. However, the number of necessary sessions can vary depending on the depth and shape of the scar, as well as the patient's skin condition. We will guide you to the most suitable plan through a consultation.
Is a recovery period needed after the treatment?
Redness may occur on the treated area, but it's usually not significant enough to disrupt daily activities. The time it takes for the redness to completely subside can vary among individuals.
What is the difference between New Skin Needle Treatment and Multi-New Skin Needle Treatment?
New Skin Needle Treatment is a procedure that severs the fibrous tissue beneath the scar, helping the scar's base to rise naturally. Multi-New Skin Needle Treatment, on the other hand, includes a delicate process of refining the edges around indented scars, in addition to the basic principle of New Skin Needle Treatment. When applied to deep scars with clear borders, it can achieve a more natural improvement.
Is there a risk that the scar might look wider if the skin is refined with the shouldering technique of Multi-New Skin Needle Treatment?
Shouldering is a process of gently refining the distinct edges of a scar. This does not enlarge the scar, but rather helps the indented area fill in more harmoniously by guiding it to blend naturally with the surrounding skin. Therefore, there is little concern that the scar will appear larger.
